Setting up form questions

The Form Questions consists standard questions and custom questions. If the standard questions doesn't meet your needs, you can add custom questions.

The following are the questions types you can choose when adding a custom question:
  • Home phone number
  • Mobile phone number
  • Short answer: a single-line text box.
  • Long answer: a multiple-line text box.
  • Radio button: select one choice within a set of options.
  • Date and time
  • Gender
  1. To add custom questions, click Add question and then click your desired question type.
  2. To edit the question, click the Edit icon.
  3. To delete a question, click the Delete icon.
  4. To copy a question, click the Copy icon.
  5. To make answering a question mandatory, click the Edit icon and then select the Required checkbox.
