Customer waivers report

This report displays a list of customers and their waivers, with details such as waiver due date, waiver status, receipt number, and description of the activity or membership package where the waiver is attached.

Clicking the Recall / Schedule a saved report definition button at the top of the page takes you to the Report Definitions page, where you can select a previously saved report definition.

 

The following fields are available on the Customer Waivers page:

The following filters are available on the Customer Waivers Report page: Waiver Type, Customer, Customer Type, Geographic Area, Age Range (of customer), Waiver, Waiver Date Range, Transaction Date/Time, Activity, Package, Program, and Alternate Key Value.

 

If the Obligatory waiver waiver type is selected, then some filters are hidden and the report includes the following columns:

  • Customer Name
  • Waiver Name
  • Signed By
  • Signed On
  • Status

Options

Print Detailed Waivers

Checkbox

Select this option to print full copies of all included waivers as part of the report, including the following details:

  • Customer Name

  • Receipt Number

  • Description

  • Status

  • Due Date

  • Waiver Text

Residency

Dropdown list

Include enrollees on the list based on their residency status:

  • Active Residents (have current residency status)

  • Non-Residents (do not have residency status)

  • Expired Residents (have residency status, but it has expired)

  • All Customers

Waiver Status

Dropdown list

Option to include customers on the list based on their waiver status, as selected from Incomplete, Completed, or Completed + Incomplete.

Output type

Dropdown list

The output type or format of the report, as selected from a predefined list.

 

The available output types are Adobe Acrobat Reader, HTML, and Microsoft Excel.

 

When the ‘Print Detailed Waivers?’ setting is NOT selected, the Microsoft Excel (Tabular Format) option is displayed.
