You can add a custom question to your activity that will be asked of all activity registrants.
Go to Administration > Registration Settings > Activities.
Search for and select the activity that you want to modify.
Click Change Custom Questions.
Choose one of the following:
To add a custom question to the activity:
Click Add New Question.
Search for the custom question.
Click the Title of the custom question.
To modify a custom question that is already linked to the activity, click the Question Name of the question.
Enter the Question Order.
Note: This determines the order this question will be asked relative to other custom questions linked to the activity.
Select whether or not the question is Required.
Note: Leaving the field blank will follow the original setting in the custom question record.
To specify that the question only needs to be asked up until a specific date, enter the date in the Do Not Show After box.
To specify that answers to the question cannot be modified past a specific date, enter the date in the Prevent Editing of Answer After box.
In the Override Question Answers section (if applicable):
Select Show for each possible answer to display for the activity.
Select which of the possible answers will be the default.
To specify that there is a number of times that a custom question multiple choice answer can be used before it is no longer available for selection for the activity, enter the number in the corresponding Activity Quota box.
Click Save.
