Adding or modifying a checklist item or an obligatory waiver

To allow organizations to configure and display waivers and information, the system supports obligatory waivers and checklist items.

 

Obligatory Waiver

An obligatory waiver does not associate with any transaction and all customers in your organization must sign. An obligatory waiver can be signed during a CUI login or on the admin side on the customer account functions page.

Note that customers can now sign an obligatory waiver only on behalf of themselves or minors in their family (the Administration Home > System settings > Configuration General page > Minor age setting determines whether a customer is a minor).

 

To create or edit an obligatory waiver:

    1. Under the Administration Home > System Settings > Checklist Items page > Obligatory Waivers section, click Add New to create an obligatory waiver or select an existing obligatory waiver to made necessary changes.

    2. In the Description field, enter the name of the waiver.

    3. To activate the obligatory waiver, set the waiver Status to Active.

    4. In the Waiver Text field, enter complete waiver text.

    5. In the Attached Document field, select an attachment if required.

    6. In the CUI Digital Signature dropdown, select if the waiver should be signed in the customer (public access) site by:

    7. To require customers within an age range to sign an obligatory waiver, in Required for customers aged between setting, enter the age range.

    8. Click Save.

 

Transaction Checklist

Checklists allow you to track waivers and other information in the registration or booking process. You can attach checklist items to activity, daycare program, membership package, or facility/equipment event type checklists. Waivers may be signed online or on the admin side on the customer account functions page. For detailed information about checklist items, click here.

Note: To modify a facility/equipment event type checklist item, first click Checklist and then click the Description link of the checklist item.

    1. On the Change Checklist Item Attachment page, if you are adding a new checklist item select the Item type from the Checklist Item section.

    2. In the Checklist Item Overrides section, select the Include Condition to specify when this checklist item will apply to a customer.

    3. To require the waiver to be signed by the customer prior to the due date (as defined below), select Required before due date?

    4. Enter the number of days prior to activity or rental start that the waiver is required in the Due nn Days Prior to Start box.

    5. To display the waiver on the customer site, click Show Item Online?

    6. To allow completion of the waiver online, click Can be Signed Online?

    7. Note: If Show Item Online? is selected but Can be Signed Online? is disabled, then customers will be able to view the checklist item on the public access site but will not be able to check the item off themselves. Only staff users will be able to check off this item. This applies to both waivers and information.

    8. To require the waiver to be signed by the customer before the transaction can be completed, click Required Before Completing Transactions?

    9. To display the waiver on the Assign Participants page on the customer site, click Show Item Online on Enrollment Page?

Note: If this checkbox is cleared and the Show Item Online checkbox is selected, the waiver will be displayed on the Confirm Order and Submit Payment page instead.

    1. In the CUI Digital Signature dropdown, select if the waiver should be signed in the customer (public access) site by:

    2. To show a signature line on the customer site, click Show Signature Line?

    3. Enter the number of days for which the waiver is valid in the Waiver Duration box.

    4. To print both the payer and the customer on the waiver and also display their name on the Transaction Checklist page for customers, click Print Payer and Customer on Waiver?

    5. Click Save.
